Preregistration Form for Youth Programs
Form to hold place in program.
Congratulations on your decision to enroll on one of our life-affirming programs!
Because many of our programs take place within the wilderness, and because we design our programs to meet the needs of the specific individuals who enroll in them, there are several stages to our registration process:
1. Submit this preregistration form.
2. Submit the preregistration deposit and/or financial aid request. Deposit amounts are listed on each program page. If you'd like, you can include a copy of the preregistration form with your deposit, but it's not necessary. Just indicate in the memo line the participant's name & program. Payments can be made by check to: Registrar, Rite of Passage Journeys, PO Box 27559, Seattle, WA 98165-2559, or by credit card (425) 485-7396. Your spot is now reserved.
3. Upon receipt of completed preregistration, we will email you a complete registration packet with instructions. Please complete and return this packet to us within one month.
4. We will contact you within 2 weeks to confirm receipt, follow up with any questions we may have, and confirm program registration. (In the case of day-long or retreat-center-based programs, a review of your materials before confirmation of registration is unnecessary. Simply return the registration questionnaires, and your spot is confirmed).

Again, preregistration is not complete until we have received your payment.
In the case of cancellation more than one month before the program, program tuition minus deposit will be refunded. If a participant withdraws after the start of a program, no refund will be issued. In some cases, credit towards a future program will be allowed.
