Preregistration Form for Counselor Camp
Form to hold place in program.
Welcome to Counselor Camp 2012: Realize Recovery
We are currently accepting preregistrations for this upcoming conference. There are several steps to the registration process:
1. Submit this pre-registration form.
2. Submit payment to reserve your spot. Payments can be made by check to: Registrar, Rite of Passage Journeys, PO Box 27559, Seattle, WA 98165, or via credit card (425) 485-7396. Write checks out to "Rite of Passage Journeys." Your spot is now reserved.
3. Upon receipt of completed preregistration, we will email you a confirmation.
4. We will contact you two weeks before the conference with complete information on the gathering.
Throughout this process please feel free to email or call with any questions you have.
Again, preregistration is not complete until we have received your payment.
In the case of cancellation more than one month before the conference, half of your payment will be refunded. If a participant withdraws less then a month before the conference, no refund will be issued. In some cases, credit towards a future Counselor Camp will be allowed.
