This is a resource page for you as you prepare to take up the role of guiding at Rite of Passage Journeys and instructions for completing any incomplete paperwork.
Employment Documents
Please upload the attached forms via your Linseed profile by using the same Linseed link provided for your guide application. You can upload the forms as Attachments. As noted in the contract, there is no need to attach a void check on the Direct Deposit Form. If you have any trouble uploading this way, you can upload via Dropbox here but please try the other way first.
Tax and Payroll Forms (For Program Coordinator & Program Mentor Guides):
These links are to fillable PDF forms on the IRS website. Fill them out, then save them as a PDF to your computer, and upload them to our Dropbox. Here are links to the forms:
W-4 (Fillable form on IRS website—fill out, save as PDF and upload)
I-9 (Fillable form on IRS website—fill out, save as PDF and upload)
Required Certifications (All Guides/Apprentices):
Scan or take a picture of your required certs and Driver’s License and upload them following the directions above:
Wilderness Medicine Certification Cards (both WFA/WFR/WEMT and CPR cards)
Driver’s license
Food Handler Card (can be done during prep days)
If you need to re-certify for CPR or Food Card, here are the links:
Food Worker Permit (Upload the pdf they email you when you complete the online course)
CPR Course (Upload the pdf they email you when you complete the online course - if you took a WFR or WFA this is included in your course)
Health and Personal Info Form (All Guides/Apprentices):
Please link to the Health Form to fill out your current contact and up-to-date health and diet info.
Acknowledgement of Policies and Procedures (All Guides/Apprentices):
Once you have read the Staff Handbook and Policies and Procedures documents linked below, please sign this Acknowledgement of Policies and Procedures document
